Rules & Restrictions:
You may receive an unlimited number of gift certificates for patients you refer to our practice, however, those patients must schedule and keep an appointment for treatment in our office ("treatment" includes exams). A maximum of 2 certificates will be granted per family referred (i.e., if you refer a family of 5, you will be given a maximum of 2 gift certificates for that referral).
